Facility Rental/Permit Application
    This NCR form is for groups wanting to reserve a pavilion, gazebo, ballfield, or rent a gym, the
    TYC, or other city facility.  Depending on the use, one also needs either the Facility Use
    Agreement or the Event Permit below.
Facility Use Agreement
    This form accompanies the Facility Rental/Permit Application form (above).  This is a
    waiver/release form which has the signer responsible for the group.  Use this form for things such
    as family picnics, company barbeques, small private parties, etc.
Event Permit
    This form accompanies the Facility Rental/Permit Application form (above).  This is used to
    transfer all responsibility and liability from the City to the group responsible for the event.  Groups
    using this form would be larger parties, any kind of special event, or an activity in which the public
    is openly invited or encouraged to 'stop by'.  The Event Permit form also requires a certificate of
    insurance from the group which lists the City as additionally insured.  Your insurance company
    generally does NOT charge more for this service.
